I am sick and tired of writing articles about obesity

It’s been a big week for obesity researchers et al.

This week we learned that America blows $147 billion a year on medical treatments related to obesity.

And we also learned that the CDC has developed America’s Official Obesity Solution.

And finally, we learned that thinnies hate fatties. And that fatties have lawyers writing books about how obesity isn’t a medical problem and that all of this anti-obesity talk is discrimination.

Oops.

Maybe I shouldn’t have said fatties.

(please don’t sue me…please don’t sue me…please don’t sue me…please don’t sue me…)

But before I go and start my long weekend of sun & fun, I have one more uplifting piece of obesity research for you.

Rates Of Severe Childhood Obesity Have Tripled

ScienceDaily (July 30, 2009) — Rates of severe childhood obesity have tripled in the last 25 years, putting many children at risk for diabetes and heart disease, according to a report in Academic Pediatrics.

“Children are not only becoming obese, but becoming severely obese, which impacts their overall health,” said Joseph Skelton, M.D., lead author and director of the Brenner FIT (Families in Training) Program. “These findings reinforce the fact that medically-based programs to treat obesity are needed throughout the United States and insurance companies should be encouraged to cover this care.”

Successful Fitness Motivation or "how I finally whipped my lazy butt into shape"

Let’s be honest.

We all know what it takes to be in good shape

  • Exercise daily
  • Eat healthy, nutritious food
  • Stay hydrated
  • Reduce your stress
  • Get 8 hours of sleep
  • Quit smoking, Quit McDonalds, Quit drinking gallons of Coke, etc….

And for some lucky people, working out and living a healthy lifestyle come naturally. They Just Do It™

However, for the rest of us, the effort required to “get into shape” takes too much…..effort.

  • It costs too much
  • It’s too hard
  • I don’t know what to do
  • Fitness people are jerks
  • People will look at me
  • People won’t look at me
  • I’m too fat
  • I don’t want to get all sweaty
  • I don’t have enough time
  • and it’s still too hard

So, we give up, give in and get out of shape.

Your biggest impediment to getting in shape isn’t a lack of money or knowledge or natural ability.

It’s your emotions.

It’s the beliefs you hold that exercise is too hard and diets don’t work and you have fat DNA.

And I am here to tell you that until you change these beliefs and the emotions that come with them, you will never, ever get in shape.

In fact, every single day, I work with people who are desperate to get in shape…but they don’t believe they can.

But, lucky for them, I do believe. I believe because I have seen it happen over and over again.

So, I lend them my confidence and my emotions.

True, I work them hard and I yell at them for breaking their diet. But, I always make them understand that they can get in shape and that I am going to make it happen.

And like magic, when they start to believe, they start to improve.

For some people, it’s right away. For others, it takes a few weeks.

But eventually, my beliefs and emotions become their beliefs and emotions.

And it’s those emotions and belief in themselves that drives the improvement in fitness.

But, let’s say you don’t have access to a charging bull or don’t want to pay for the airline ticket to fly me to your home town.

What do you do?

You need to make 3 Decisions

  1. What are you going to focus on?
  2. What does getting in shape mean to you?
  3. What actions are you going to take?

FOCUS

  • Are you going to focus on your past failures? – NO
  • Or, are you going to focus on your fantastically fit future self? – YES

What you focus on creates emotions. We want/need positive emotions

MEANING

  • What does living a healthy life mean to you?
  • Does it MEAN fatigue/pain/sweat/expense/pain/lack of confidence/pain??? – NO
  • Or, does it mean feeling great, looking great and actually being great? – YES
  • Is it a punishment or is it a reward?

Meaning creates emotions. We want/need positive emotions.

ACTION

  • Your emotions WILL control your actions…positive emotions = positive actions
  • And, when you are ready to take positive action, you will find that there are more than enough resources available to transform your body from fat to fit.
  • But, of course, you should start by looking through the Health Habits archives.

America's "Official" Obesity Solution

According to Health Secretary Kathleen Sebelius, the US medical system spends around twice as much treating obesity related illnesses than it does on cancer.

Obesity Related Illnesses: $147 billion

Cancer (all types): $93 billion

And how, you may ask, is that $147 billion being spent?

  • Prevention…….Nope
  • Drugs & Surgery…….Yep

But, not anymore.

A few years back, the CDC initiated the Common Community Measures for Obesity Prevention Project (aka the Measures Project).

The goal of the Measures Project was to identify and recommend a set of obesity prevention strategies and corresponding suggested measurements that local governments and communities can use to plan, implement, and monitor initiatives to prevent obesity.

The Measures Project process was guided by expert opinion and included a systematic review of the published scientific literature, resulting in the adoption of 24 recommended environmental and policy level strategies to prevent obesity.

This report presents the first set of comprehensive recommendations published by CDC to promote healthy eating and active living and reduce the prevalence of obesity in the United States. This report describes each of the recommended strategies, summarizes available evidence regarding their effectiveness, and presents a suggested measurement for each strategy that communities can use to assess implementation and track progress over time.

Translation: This is the first big gov’t approach to obesity prevention through lifestyle modification.

And here are the 24 strategies.

Strategies to Promote the Availability of Affordable Healthy Food and Beverages

Strategy 1

Communities should increase availability of healthier food and beverage choices in public service venues.

Suggested measurement
A policy exists to apply nutrition standards that are consistent with the dietary guidelines for Americans to all food sold (e.g., meal menus and vending machines) within local government facilities in a local jurisdiction or on public school campuses during the school day within the largest school district in a local jurisdiction.

Translation: Stop selling junk food in schools & public facilities

Health Habits Comment: What constitutes healthy food? Who determines what is healthy food? CDC? USDA?

Communities should improve availability of affordable healthier food and beverage choices in public service venues.

Suggested measurement

Translation: City planning, taxation & bylaws (ex. LA has banned new fast food restaurants in low income neighborhoods)

Communities should provide incentives to food retailers to locate in and/or offer healthier food and beverage choices in underserved areas.

Suggested measurement
Local government offers at least one incentive to new and/or existing food retailers to offer healthier food and beverage choices in underserved areas.

Communities should improve availability of mechanisms for purchasing foods from farms.

Suggested measurement
The total annual number of farmer-days at farmers’ markets per 10,000 residents within a local jurisdiction.

Communities should restrict availability of less healthy foods and beverages in public service venues.

Suggested measurement
A policy exists that prohibits the sale of less healthy foods and beverages (as defined by IOM [Institute of Medicine]) within local government facilities in a local jurisdiction or on public school campuses during the school day within the largest school district in a local jurisdiction.

Strategy 8

Communities should institute smaller portion size options in public service venues.

Suggested measurement
Local government has a policy to limit the portion size of any entree (including sandwiches and entrée salads) by either reducing the standard portion size of entrees or offering smaller portion sizes in addition to standard portion sizes within local government facilities within a local jurisdiction.

Translation: The opposite of Super-Size Me.

Strategy 9

Communities should limit advertisements of less healthy foods and beverages.

Suggested measurement
A policy exists that limits advertising and promotion of less healthy foods and beverages within local government facilities in a local jurisdiction or on public school campuses during the school day within the largest school district in a local jurisdiction.

Translation: Media bans on public property

Strategy 10

Communities should discourage consumption of sugar-sweetened beverages.

Suggested measurement
Licensed child care facilities within the local jurisdiction are required to ban sugar-sweetened beverages, including flavored/sweetened milk and limit the portion size of 100% juice.

Translation: Replace fruit drinks with fruit juice and chocolate milk with plain milk or even…water

Strategy to Encourage Breastfeeding

Strategy 11

Communities should increase support for breastfeeding.

Suggested measurement
Local government has a policy requiring local government facilities to provide breastfeeding accommodations for employees that include both time and private space for breastfeeding during working hours.

Strategies to Encourage Physical Activity or Limit Sedentary Activity Among Children and Youth

Strategy 12

Communities should require physical education in schools.

Suggested measurement
The largest school district located within the local jurisdiction has a policy that requires a minimum of 150 minutes per week of PE in public elementary schools and a minimum of 225 minutes per week of PE in public middle schools and high schools throughout the school year (as recommended by the National Association of Sports and Physical Education).

Communities should increase the amount of physical activity in PE programs in schools.

Suggested measurement
The largest school district located within the local jurisdiction has a policy that requires K–12 students to be physically active for at least 50% of time spent in PE classes in public schools.

Communities should increase opportunities for extracurricular physical activity.

Suggested measurement
The percentage of public schools within the largest school district in a local jurisdiction that allow the use of their athletic facilities by the public during non-school hours on a regular basis.

Strategy 15

Communities should reduce screen time in public service venues.

Suggested measurement
Licensed child care facilities within the local jurisdiction are required to limit screen viewing time to no more than 2 hours per day for children aged ≥2 years.

Health Habits Comment: How about 0 hours of tv? How about reading a book to the kids, or arts & crafts, or playing a game. Sheesh!

Communities should improve access to outdoor recreational facilities.

Suggested measurement
The percentage of residential parcels within a local jurisdiction that are located within a half-mile network distance of at least one outdoor public recreational facility.

Strategy 17

Communities should enhance infrastructure supporting bicycling.

Suggested measurement
Total miles of designated shared-use paths and bike lanes relative to the total street miles (excluding limited access highways) that are maintained by a local jurisdiction.

Communities should enhance infrastructure supporting walking.

Suggested measurement
Total miles of paved sidewalks relative to the total street miles (excluding limited access highways) that are maintained by a local jurisdiction.

The largest school district in the local jurisdiction has a policy that supports locating new schools, and/or repairing or expanding existing schools, within easy walking or biking distance of residential areas.

Strategy 20

Communities should improve access to public transportation.

Suggested measurement
The percentage of residential and commercial parcels in a local jurisdiction that are located either within a quarter-mile network distance of at least one bus stop or within a half-mile network distance of at least one train stop (including commuter and passenger trains, light rail, subways, and street cars).

Health Habits Comment: How does public transit directly affect fitness/obesity?

Strategy 21

Communities should zone for mixed use development.

Suggested measurement
Percentage of zoned land area (in acres) within a local jurisdiction that is zoned for mixed use that specifically combines residential land use with one or more commercial, institutional, or other public land uses.

Translation: Create self sufficient “villages” within a community. This way you can walk to the grocery store instead of driving to the mall. Livable city concept.

Strategy 22

Communities should enhance personal safety in areas where persons are or could be physically active.

Suggested measurement
The number of vacant or abandoned buildings (residential and commercial) relative to the total number of buildings located within a local jurisdiction.

Translation: In urban areas, walking at night can be a very real threat to your health.

Health Habits Comment: Violent crime is not a quick or simple fix. No idea how they plan to enhance personal safety in dangerous neighborhoods.

Strategy 23

Communities should enhance traffic safety in areas where persons are or could be physically active.

Suggested measurement
Local government has a policy for designing and operating streets with safe access for all users which includes at least one element suggested by the national complete streets coalition

Translation: Automobiles give up some space to pedestrians, bikes & transit.

Strategy to Encourage Communities to Organize for Change

Strategy 24

Communities should participate in community coalitions or partnerships to address obesity.

Suggested measurement
Local government is an active member of at least one coalition or partnership that aims to promote environmental and policy change to promote active living and/or healthy eating (excluding personal health programs such as health fairs).

Translation: Big federal programs won’t work. Grassroots is the way to go.

Now all we need is some of those big federal health care dollars to come rolling in.

Does Coffee Make You Fat?

I just finished reading Why Diet and Exercise Fail…and I am ticked off.

starbucks IV addictTicked off because author Daniel Matthew Korn is making me re-think my morning cup of Gold Coast.

Korn believes that long term caffeine consumption, when combined with a typical Western Diet, is akin to throwing gas on your obesity bonfire.

Or, as Daniel says, “long term caffeine use, in combination with other dietary factors can contribute to lowering our ability to use our stored body-fat and interfere with our storage of nutrients”.

Why would coffee lead to obesity?

Theory #1

  • Long term caffeine use can lead to elevated stress hormones (ie Cortisol)
  • And heightened cortisol levels is associated with obesity

Theory #2

  • Chronically elevated cortisol levels results in increased feelings of hunger
  • And obviously hunger leads to eating and drinking more coffee and eating more food and…

Theory #3

  • Chronic caffeine use may interfere with your sleep patterns
  • And sleep deprivation is associated with obesity

Korn’s Conclusion

Chronic caffeine use contributes to obesity because:

  • it increases your level of stress hormones
  • which messes with your brain chemistry
  • and increases your hunger
  • causing you to eat more crappy processed foods
  • while you lay in bed tossing and turning because you can’t sleep

My Conclusion

As Korn mentions, there is very little research into the direct effect caffeine has on metabolism and obesity.

This leaves his theory open to attack.

I can hear the comments already – Association and/or Correlation is not Causation.

And they are right. Korn is making some assumptions.

And good for him. Out of these assumptions, researchers can design studies to test if caffeine has a direct effect upon obesity.

Until then, you can:

  1. Ignore his caffeine/obesity theory as unproven
  2. Or, test the theory on yourself. Sadly, that is the route I am taking…starting tomorrow.

America's Obesity Epidemic costs $147 billion

America's Obesity Epidemic

According to this study,

  • Americans spend $147 billion per year on “medical” obesity treatments,
  • This spending on obesity treatments eats up 9.1% of America’s total health care spending,
  • Annual medical costs due to obesity have doubled in less than a decade,
  • Between 1998 and 2006, the prevalence of American obesity swelled by 37%.
  • The results also showed that an obese person has $1,429 per year more medical costs, or about 42 percent more costs, than someone of normal weight.
  • Costs for an obese Medicare recipient are even greater.

Much of the costs to Medicare are a result of the added prescription drug benefit. The results reveal that Medicare prescription drug payments for obese individuals are roughly $600 more per year than drug payments for normal weight beneficiaries.

The researchers also found that 8.5 percent of Medicare expenditures, 11.8 percent of Medicaid expenditures, and 12.9 percent of private payer expenditures are attributable to obesity.

According to lead author Dr. Eric Finkelstein, “the medical costs attributable to obesity are almost entirely a result of costs generated from treating the diseases that obesity promotes“.

Insulin Resistance, Metabolic Syndrome, Diabetes, Cardiovascular Disease….

“Obesity will continue to impose a significant burden on the health care system as long as the prevalence of obesity remains high.”

This study was presented earlier today at the “Weight of the Nation” conference, a three-day meeting of public health experts, policy leaders, and researchers being held in Washington, D.C.

The CDC will discuss the study findings along with new recommendations designed to prevent and reduce the impact obesity has upon communities at the “Weight of the Nation” conference, a three-day meeting of public health experts, policy leaders, and researchers being held today in Washington, D.C.

But, let’s not wait for the CDC discussion….

How can America reduce the $147 billion spent each year “treating the diseases that obesity promotes”?

  • More bariatric surgeries?
  • Modify the current taxes/subsidies applied to food production/distribution?
  • Mandatory P.E. in all schools?
  • Community fitness initiatives?
  • Massive PR campaign designed to shame the lazy & praise the physically fit
  • Tax breaks for Pharma research into obesity treatments
  • Mandatory military bootcamps
  • Amphetamines added to the water supply (sorry, Coca-Cola supply is more appropriate)

Mango-Jalapeno Crabcakes (Gluten Free!)

Mango-Jalapeno Crab Cakes

Like any true Irishman, I love me some fishcakes. So much so that I’ve nearly an arsenal of fishcake recipes! In my days, crab in Ireland was pretty much served as a ’salad’ mixed with mayo and Mary Rose Sauce or in a casserole. My first experience with a crab cake was on my first trip stateside, in Martha’s Vineyard and I’ve been hooked ever since. Traditionally they’re made with mayo and sometimes even butter but you know me, that’s not my bag (unless you make homemade mayo–click Mary Rose link above). But really, there’s nothing worse than ordering a crab cake that’s full of mayo and not so much crab, right? You won’t have to worry about that with this recipe! Also good to note, Dungeness crab meat is rated an Eco-Best choice.

Your Body-Fat Wants to Kill You

Researchers know that your body-fat is not simply a storage place for excess calories, but is in fact an active tissue that secretes multiple compounds.

It communicates with other tissues, including the liver, muscles, pancreas and the brain.

In a “normal” person, communication between body-fat and other body tissues helps to optimize metabolism keep body weight stable.

However, in obese individuals:

  • Fat cells become stressed as they over-grow in order to store the increasing amounts of fat
  • This excessive cell growth may cause decreased oxygen delivery into the tissue;
  • Individual cells may die (at least in mouse models),
  • And fat tissue inflammation ensues.

This leads to mis-communication between fat cells and the other body tissues responsible for metabolism.

And this mis-communication leads to lifestyle diseases like type 2 diabetes and cardiovascular disease.

The Science

To understand this cellular mis-communication better, the researchers began by collecting fat samples from people undergoing abdominal surgery.

Utilizing their giant brains, the scientists were able to identify a signalling pathway that is operational in intra-abdominal or belly fat.

They compared this signalling pathway with the pathways found in samples collected from:

  • lean individuals
  • and subjects whose body-fat was classified as peripheral or subcutaneous.

The researchers discovered that the signaling pathway was more active depending on the amount of fat accumulation in the abdomen, and that it correlated with multiple biochemical markers for increased cardio-metabolic risk.

Moreover, the expression of one of the upstream signaling components, a protein called ASK1, predicts whole-body insulin resistance (an endocrine abnormality that is strongly tied to diabetes and cardiovascular disease), independent of other traditional risk factors.

Researchers also demonstrated that although non-fat cells within adipose tissue express most of this protein in lean persons, the adipocytes themselves increase its expression by more than four-fold in abdominally-obese persons.

Conclusion

In lean persons, a normal cellular signaling process works as nature designed.

In obese persons, that signaling process is all screwed up and often leads to insulin resistance, diabetes and cardiovascular disease.

Having trouble sticking to your diet?

According to a new study, heart disease, hypertension, diabetes and obesity are on the rise in Canadians of all ages. (same goes for you non-Canadians)

It’s so bad that for the first time in 2 centuries, Canadians can expect to die earlier than their parents.

And why, you may ask are Canadians dropping like flys on a bug zapper?

Because they are fat.

The authors of the study found that hypertension and diabetes are increasing most in those who were obese or overweight.

I, Bodybuilder….Fitness Marketing 101

Nobody does a better job marketing to amateur bodybuilders than the guys at T-Muscle.

And they may have outdone themselves with this program.

Just released this morning, people are already going nuts.

They are lining up to buy a supplement that costs 80USD and may only last for two weeks.

Prorate that over a year and we’re talking just over 2000USD.

And they’re begging to buy it.

Begging.

OH MY GOD THIS IS SO EXCITING I WAS LITERALLY SCREAMING AT THE TOP OF MY LUNGS EVERY 10 SECONDS WHILE READING THE ARTICLE.

So, my question is…

What do we need to do to get the general public this interested in their own health & fitness?

What buttons need to be pushed?

Imagine how quickly America’s obesity epidemic would be turned around if everyone got this excited about a new exercise  nutrition program.

300 million fitness junkies.
